What kind of conditions do you treat?

Do you accept insurance?

We are an out-of-network provider, and our clients pay out of pocket for our services. However, suppose your healthcare plan has out-of-network physical therapy benefits. In that case, you may submit your claims for potential reimbursement from your insurance provider. 

We will provide you with a superbill, a detailed invoice that lists the physical therapy services you received. This document contains all the necessary information to create a healthcare claim, which you can then submit to your insurance company for potential reimbursement. It's important to note that while a superbill does not guarantee reimbursement, it increases the likelihood of it.

Before starting physical therapy treatment, it's a good idea to contact your insurance's member services for instructions on submitting claims and to understand any policy requirements.

As an out-of-network provider, we want to make it clear that we cannot guarantee insurance reimbursement. Furthermore, if you are a self-pay client, we are unable to handle insurance interactions on your behalf. Our services are limited to providing you with the superbill.

We are unable to assist with authorizations, approvals, appeals, or any other specific requests from your insurance plan. Understanding these limitations will help you manage your own expectations.

Can I use my Flex Spending Account (FSA) or Health Savings Account (HSA) to pay for your services?

Yes, you can use your FSA or HSA account to pay for your physical therapy sessions.

Do I need a prescription/referral from my doctor to receive physical therapy treatment?

Florida allows you to start physical therapy without a doctor's referral for the first 30 days. If your treatment needs to go beyond 30 days, our Recovery Packages include a complimentary consultation with a Sports Medicine Physician, so you're covered.

How much will a session or evaluation cost?

Our Detailed Evaluation is $195. Depending on the number of sessions included in your Recovery Package, the price of follow-up sessions ranges from $187 to $135 per visit.   

Do you offer discounts for multiple sessions?

The more visits in your Recovery Package, the more discounted they get. You get an additional 10% discount if you pay for your package in full. 

What is your cancellation policy?

Plyogenix has a 24-hour cancellation policy. You need to cancel or reschedule your appointment at least one day in advance. If you cancel within 24 hours of your appointment, you'll be charged a late fee equal to half the session rate.

What should I expect from the physical therapy evaluation?

Our Detailed Evaluation lasts 90 minutes and includes three key phases:

1. Detailed background and symptom exploration: We take the time to understand your story, including your pain, goals, medical/injury history, likes and dislikes, and what led you to seek our help.

2. Motion analysis and performance testing: Using what we learned in phase 1, we conduct a tailored evaluation to see how your body functions and find areas for improvement or relief.

3. Review findings and develop a recovery plan: We discuss our findings and recommend specific movements, exercises, and lifestyle changes to help you improve.

Together, we create a roadmap to recovery that fits your lifestyle and is designed to get you back to 100% and beyond. 

How many physical therapy visits will I need?

The number of physical therapy sessions you need depends on what your physical therapist finds during your Detailed Evaluation and what your goals are.

Someone aiming to reduce pain for daily activities will need a different number of visits compared to someone recovering from surgery and wanting to return to high-level sports.

At the end of your Detailed Evaluation, your physical therapist will discuss in detail how many sessions you'll need and how long it will take to reach your goals.

How long are Physical Therapy Evaluations and Follow-up sessions?

Our Detailed Evaluation is a 90-minute session; every follow-up Recovery Session is 60 minutes.

What do I need to bring for my Physical Therapy Evaluation?

If you have any medical records like MRI/X-ray reports, surgical reports, or medication lists, please bring them to your appointment. You can bring printed copies or digital versions. Also, don't forget to bring a water bottle because you'll probably work up a sweat during your session. ;)

What type of clothing should I wear?

Wear comfortable clothes you can move in, like gym shorts and a T-shirt or tank top. We recommend wearing sneakers and socks. If you play a sport and have special equipment, clothing, or shoes, bring those, too. It can help your physical therapist better understand your unique needs.
